Initially, the unnamed man came to the emergency unit (ER) at the Prof.Dr.I.G.N.G Ngoerah General Hospital, Bali, Indonesia, after experiencing pelvic pain and weight loss of around 8 kg in two months. Referring to physical and laboratory examinations, the doctor found a mass in the man's pelvis.

Apart from that, the laboratory examination also found anemia, leukocytosis and decreased kidney function. The patient then underwent radical nephrectomy and pathological anatomical examination.

As a result, the man turned out to have high-grade carcinoma, a type of polymorphic renal cell carcinoma (RCC) in the left kidney. This condition occurs when abnormal cells begin to divide and grow uncontrollably.

Two weeks later, the patient experienced pain and prolonged erection for one week. The patient told the doctor that the erection he experienced occurred randomly and was not due to any sexual stimulation.

Then referring to the physical examination, the doctor found an erection in the penile shaft accompanied by weakness in the glans penis. The patient's clinical diagnosis was non-ischemic priapism. Early surgeons tried to drain blood from the penis in an effort to ease the erection. However, these experimental efforts did not produce results.

The doctor then installs a bypass to help blood flow in the penis. After some further examination, doctors discovered the cancer had spread to his penis. Doctors decided to amputate the man's penis to stop the spread of the disease.

After surgery, the patient is then treated with radiotherapy and chemotherapy. Three months later the doctor said the man had no abnormalities in the urinary tract and no significant complaints.
